All you have to do is fill out the form with your first name, last name and phone number, print the confirmation page, and pre-pay in store until 13 April. From 15 April - 13 May 2011, pick up  your copy of Harry Potter and the Deathly Hollows, Part 1 on DVD for just $14.99, or Blu-ray Combo Pack for just $21.99.

Movies not picked up before the end of business on 13 May 2011 will be donated to local children's hospitals.
