So how do you get your rebate?  Purchase any specially marked Physician's Formula product until 30th November 2011. Complete the rebate form and mail it along with the mail in rebate label that is attached to your product and your original sales receipt with purchase price circled. The rebate must be postmarked by 15th December, 2011.
