Purchase any of the above items with qualifying barcodes, fill out the appropriate mail in rebate form and mail it along with your original sales receipt. You should receive your rebate in no more than 10 weeks.

I took advantage of these rebates the last time around. My refund came within about two months. The "Eye Booster ™ 2-in-1 Lash Boosting Eyeliner + Serum" is a fantastic product!

I sent in for the $7 rebate on the Organic Mascara. I downloaded and filled out the form, mailed the receipt in May 2012 and never received a refund. When I enquired at Physicians Formula customer service, Erica told me the Canadian redemption house had no record of my request and could not give me the rebate. Also in future I was to make copies of everything I sent to them for rebates; which will not happen as I will not deal with them again. Not only did I not get the $7 rebate but wasted my time and money on mailing it. So much for a good deal!

Recently purchased Physian's Formula Sexy Booster Lip Gloss. Was supposed to have a $5. mail in rebate. Could not access the site . Thanks a lot
